Download Forms and Get Information to do a Colorado QDRO

Summary: There are only 2 forms to download in order to provide information to do a QDRO.

Here Are Two Forms to Download, Which Are Used in the QDRO Preparation Process:

1. Release and Authorization; so that the plan administrator will talk with us if we have any questions.  It is also used to request preliminary approval of the draft of the QDRO before it is sent to the court.

2.  Information Needed form.  Not all of the information requested applies to every case, however I ask that you fill out as much of it as applies to you.

Additionally, a scan of two pages of IRS Form 5329 can be downloaded.  Form 5329 is used to avoid the 10% early withdrawal penalty on withdrawals from qualified plans set up by a Colorado divorce QDRO.
